Final narrative
On December 19, 2024, an employee was walking between a scribing machine and a conveyor (approximately 6-10" gap) to go to the back end of the machine. He was then struck by a 3-inch steel tube (I-beam) that pinned his right thigh against the conveyor. He was hospitalized with crush injuries to his right thigh and right hand, with a 2-centimeter laceration to his anterior thigh and circumference abrasions. The machine was not guarded at the time.

An employee was working to build an electrical substation. He was cleaning out the inside of a fixed section of a PVC pipe on the end that was furthest from an excavator. The excavator was outside of the trench and more than 25 feet away when it moved a pipe, pinching the employee's right arm between the two pipes. The employee's forearm was fractured.
An employee was anchored to a movable piece of equipment called a "lid" with a self-retracting lifeline harness. An overhead crane lifted the equipment, causing the employee to be pulled by his harness. The employee was pulled off the walkway and swung over the wall of the kiln. His body and head struck the wall. The employee lost consciousness and sustained a laceration to the side of his head that required stitches. The employee also sustained multiple fractured ribs and fractured vertebrae.

On October 21, 2025, an employee was operating a saw when a pneumatic clamp caught their right middle finger. The employee suffered a partial amputation to the finger.
An employee completed an end cut on a metal shim and started transferring the metal shim from the saw conveyor to two saw horses in the area. The employee set one end of the shim (7 feet long x 8 inches wide, weighing 218 pounds) on one of the saw horses, then slid the other end onto the other saw horse and the shim fell from the first saw horse. The employee's left index fingertip was pinched between the saw horse and the shim, resulting in a skin avulsion. The fingertip was amputated later.
