An employee scraping dough off the extruder during an equipment changeover and tripped over a ratchet strap that was connecting the extruder to the proofer belt. She fell on her right side and fractured her right hip.

An employee was conducting preventive maintenance on a locked-out fryer exhaust on a roof. Airflow started to move the fan blades, which caused the belt to move. The employee's glove and index finger were caught by the belt and pulled into the pulley, causing an amputation to the finger.
An employee was feeding product through a crimper. The employee's left thumb entered the gap in the packaging lateral seal and the tip was partially amputated.
