An employee was assisting a forklift driver by breaking down bundles of wood pulp to fit the product onto a trailer. The employee spotted a strap on one of the loads and went to remove it by placing his left arm between the forklift's attachment and the load. The forklift driver started to move and the employee's left arm got pinned between the forklift and the load, resulting in compartment syndrome.

An employee was walking to the dock when they became entangled in a tagline. Their legs were caught in the tagline and their left foot was amputated by the tagline.

While slack was being taken off a line, a pipe shifted and struck the employee, resulting in a left ankle fracture.

An employee was moving a 55-gallon drum a few inches closer to another steel drum. The employee's thumb was caught between the two drums. The bone between the tip of the thumb and the first joint was broken, and the thumb was lacerated on the outside, from the tip to the first knuckle.

An employee was removing a ship rope from the steel hook on a buoy. The hook crushed his left big toe and second toes. The big toe was medically amputated at the base of the nail.

An employee was painting a round module when a front-end loader ran over his right leg and crushed it.

The switch truck rolled and pinned the employee against the container. The employee sustained a fractured pelvis, a ruptured spleen, and a possible spinal cord injury.

On October 16, 2025, an employee was entering a parking lot after completing their work shift when they were struck by a vehicle being operated by a customer. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was moving branches and other material when they were struck in the leg by the track of a skid-steer loader, resulting in an ankle and lower leg injury.

An employee was waiting for an operator to bring mail over to a mail sorting machine when she became pinned between the machine and a stack of pallets being pushed by a powered industrial truck (PIT). While preparing to install a new spool piece for a vapor and spent sulfuric acid line, an employee was working to ensure there was no residual product or pressure in the line before removing the 6-inch hose. The valve was cracked, and some spent acid still in the line sprayed out. The employee was standing off to the side and at a distance, but was still contacted by the hot acid, resulting in corrosive and thermal burns to their upper torso and mid-thighs of both legs.

An employee was driving a utility tractor rig when it struck the rear trailer of another utility tractor rig, which was traveling in the opposite direction. The glancing impact caused damage to the cab, and he sustained a laceration on right side of his face, a cervical fracture, two fractured left ribs, and a pelvic fracture. The employee was hospitalized.

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

An employee was standing on the outside edge of a hopper barge while watching a skid steer push ore for an excavator (with a clamshell bucket) to unload. The excavator was on a work barge. The bucket of the excavator struck the employee and caused a near-amputation of their right arm and fractures of the ulna and radius of the left arm. The right arm required surgery.

An employee was inspecting a conveyor to see why it was not running. The employee entered the the conveyor motor area, tripped on a piece of angle iron welded to the floor, and fell. The employee sustained a fractured orbital bone.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
