A group of employees were cleaning up campground jobsites at the end of the day. The injured employee was sitting on the tailgate of a work truck when the driver pulled away and drove downhill for approximately 4 to 5 feet, causing the injured employee to fall off the tailgate. The injured employee struck their head on the ground, losing consciousness. They also suffered head and neck injuries, including a brain bleed that required hospitalization.

An employee was working on a lighting junction box above the ceiling and cut an energized 277-volt circuit wire. The employee was shocked and received an electrical burn on their right hand.

An employee experienced a shock and arc flash when cleaning the inside of electrical gear that was believed to be shut off. The employee received second and third degree burns on the face, hands, and arms.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was riding a four-wheeler while moving cattle into another pen when they were thrown off the vehicle and landed on the ground. The employee sustained fractures to their upper arm, pelvis, and right leg.

An employee was moving a stand-up powered industrial truck from a warehouse to a service area. During a turn on a slight decline, the vehicle's steering chain broke and the vehicle spun. The employee fell off the vehicle and landed on his right knee, breaking his fibula.

Scheuermann Excavating, Inc.

Two employees were operating a paving machine at approximately 1 mile per hour. The paver made a sudden turn, that threw the injured employee off balance. He reached for the handhold next to him and missed, falling 6 inches off the platform to the ground. The adjustable handle for the wing on the paving machine went between his left-hand fingers with the full weight of his body. The employee suffered a closed fracture of the fourth metacarpal bone in his hand.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was standing on an A-frame ladder approximately 6 feet above the floor while he attached a suspended light fixture to the ceiling. The employee lost balance, and fell to the tiled floor below, sustaining blunt force trauma to his head.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
