An employee was troubleshooting the lubrication system for kiln cooler gear. He locked out the system and shut off the compressed air to the lubrication system. Then, he went to unclog the system by pushing the cleanout plunger on the backside of the nozzle while holding the spray bar. His left hand slipped in front of the nozzle as the injector released stored oil pressure, resulting in the injection of air and oil lubricant into his hand at 3,000 psi.

While working on top of a rail car, an employee's leg got caught on the top of the rail car. The employee fell to the rail car surface and sustained a broken tibia and fibula.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was performing maintenance on a pump flange that was in ammonium sulfate service. The line had been blocked in and drain lines were opened. The employee was making a line break when hot sulfur water (150-180 degrees) sprayed out. The employee sustained thermal burns to their chest, back, arm, and stomach.

An employee was measuring the level in a storage tank using a handheld tape gauge. When he stood up, he felt a sharp pain in his back. He was hospitalized, having suffered a lower back strain.

An employee was performing gas service work outdoors in extreme heat when they became ill. The employee was hospitalized with dehydration.

The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.
