An employee was walking in the parking lot from the office to the parts building when he slipped and fell due to winter conditions. The employee was hospitalized with fractures to the right tibia and fibula, requiring surgery.

At approximately 9:30 AM, a driver was delivering liquid nitrogen to a customer's bulk tank. He had disconnected the delivery hose and laid it on the ground. He then stepped on the hose, lost balance, rolled his ankle, resulting in a fractured right tibia.
An employee was delivering liquid carbon dioxide. A CO2 valve was open, and the release of pressure struck the employee in the chest. The employee fell to the ground and suffered a head laceration.
