An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was dismounting a pallet jack. Their left hand was on the throttle to help balance themselves. As they got down, the pallet jack drove into their leg, resulting in an injury to their Achilles tendon and hospitalization.

On August 30, 2025, an employee was operating an electric double-pallet jack. He was coming out of the new extension through the separation curtains when his pallet jack struck an electric forklift. The forklift landed on the employee's right foot, fracturing the big toe and second toe.

An employee was loading pallets of food into a trailer with an electric ride-on pallet jack. The pallet jack became stuck after the employee placed a pallet in the trailer. Once freed, it became unbalanced and pinned the employee's left foot against the trailer wall. The foot was broken.

An employee was performing work on foot and was struck by a passing forklift, resulting in a left leg fracture.

As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was operating a stand-up forklift. The forklift drove into a rack in a warehouse, and the employee's right hand was caught between the rack and an upright supporting the forklift's overhead guard. The employee suffered a fracture and laceration to the hand, just below the little finger.

An employee was cleaning a depositor machine to setup for a new product run. His right index finger was caught by moving parts in the depositor machine and the fingertip was partially amputated without bone loss.

An employee was under a label machine, working to remove a jammed label. The machine partially amputated her left index finger at the first knuckle.

An employee was operating an electric pallet jack. While standing beside the equipment, the pallet jack lurched forward, pinning the employee's leg against racking. The employee was hospitalized with a fractured right ankle.

An employee was on a top stock cart, moving items on shelving on the sales floor. The employee fell and suffered a broken left ankle. The employee was hospitalized, requiring surgery.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
