An employee was exiting a rock crusher and getting onto a 6-foot step ladder. He was holding onto the handrail while getting his feet onto the ladder. His feet were on the second rung from the top when the ladder kicked out from underneath him. He held onto the handrail momentarily and then fell 4 to 5 feet to the ground (gravel pad), breaking his left wrist and suffering a small compression fracture in his lower back. He was hospitalized.

NATIONAL MILL MAINTENANCE, LLC

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

C. Miller Drilling Inc

A service crew was pulling a pump and motor from a well. After breaking a joint of pipe loose from another joint, an employee grabbed the chain tongs to prevent the pipe from spinning in the well, so the crew could continue taking it apart. The bolt that attaches the chain to the handle of the chain tongs broke, causing the chain to swing around and strike the employee's lower right leg. The chain severely punctured his leg and fractured his tibia.

An employee was using a handheld grinder to cut a bolt. The grinding wheel broke apart and a fragment struck the employee s right hand, causing a laceration to the palm near the little finger.

Blackburn's Fabrication, Inc.

An employee was operating a brake press when the top die broke, causing the bottom die to roll out and onto the employee's legs. Both of the employee's legs were crushed below the knee, and both legs were partially amputated. The employee's left leg was also fractured above the knee.

A pressurized aircraft nose landing gear strut assembly was being disassembled. The gland nut dislodged under pressure and made contact with an employee's left hand, breaking it.

An employee was checking voltage and fuses to see where power loss was occurring. He touched a magnetic coil and was shocked.

An employee was standing on an injector head feeding chain with a rope, when the rope broke, causing him to lose his balance and fall to the ground. The employee sustained fractures to his left elbow and three ribs.

An employee was pulling drill line from the drill spool while the top drive was not set on the docking pins. The deadman was unexpectedly released which caused the top drive to fall to the drill floor from a height of 30 feet. This caused the drill line that was being pulled out and laid on the ground to retract and strike an employee. The employee suffered fractures to his legs and neck.

An employee was conducting maintenance on a pipe and was using a T-bar tool to secure the retainer cap on a pump. The employee's left index finger was caught in the rotating pump, resulting in a partial amputation.

CITGO PETROLEUM CORPORATION

An employee was separating a valve stem from an actuator stem by removing two-piece split nuts that hold it together. The actuator shaft shifted, causing the top half of the split nut to impact and pinch the employee's right thumb against the frame. The employee was hospitalized.

Turnagain Marine Construction Corporation

An employee was assisting with pre-welding setup on pile structures at the fuel dock worksite. They were working to operate a come-along to align a waler to the pile. Due to limited leverage and restricted positioning within the man basket of the crane, the employee exited the basket to manually adjust the waler from the structure itself. During the task, wave height and frequency increased. A large wave struck the area, causing the pile braces and walers to shift. The employee lost their footing and grabbed the production pile edge. Their fingers on one hand were crushed between two structural steel piles. The employee sustained an amputation to the fingers and was hospitalized.

Circle Seafoods Qalib, Inc.

An employee was in a tunnel freezer, which was being turned on for the first time. The baffling for the evaporator fan got stuck in the siding. The employee ascended a 5'9"-tall cart to reach the raised baffling panel. The raised baffling shifted and dropped, trapping the employee's arm between the aluminum and foam core baffling. The employee sustained a severe laceration to the lower forearm/upper wrist and severed ligaments. The employee was hospitalized.

An employee was engaging in daily physical training in preparation for fire season as a smokejumper. The employee sustained swelling and stiffness in their arms and was hospitalized with rhabdomyolysis.
