On July 30, 2020, at approximately 10:30 AM, a temporary employee was operating a rotary molder and adding dough in the rollers. The employee came in contact with the knife on the rollers resulting in a partial amputation to the right thumb.

A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.

An employee tripped while walking from one store register station to another. Her forehead struck a register, and she suffered fractures to two vertebrae in her neck. She was hospitalized.

Tri-Tech Laboratories

An employee was using an electric pallet jack to retrieve a chemical tote from a staging lane. He inserted the pallet jack under the tote and began to back up. After moving backward for approximately 15 feet, the employee tripped on a collapsible tote. As he fell, the pallet jack ran over his left foot. His ankle was lacerated and his foot was fractured.

An employee had been unloading tires from the back of a pickup truck. The employee went to step down out of the back of the pickup truck and stepped on the lift gate in a vertical position. The lift gate tipped and employee fell, impacting their side on the upright liftgate. The employee sustained five fractured ribs and a punctured right lung. The employee was hospitalized.

An employee fell onto a hot billet and suffered a burn to the left leg. The employee was hospitalized.

New Horizon Baking

An employee (who had recently walked through water accumulated on the floor) slipped and fell to the floor. The employee suffered a broken right wrist and right elbow and was hospitalized, requiring surgery.

An employee was performing maintenance on a production line, requiring a testing and positioning phase for the whipped cream dispensers. The employee was positioning the dispensers using wrenches when one of the sensors was triggered by the pie pans coming down the line. The depositor heads lowered and pinched the employee's left ring finger, resulting in an amputated fingertip. The whipped cream dispensers were not guarded at the time and the line was not locked out/tagged out.

During a line changeover, an employee was rinsing a depositor with water. The employee was working to remove a piece of chocolate, stuck in the machine's roller, when the roller's moving parts caught his middle finger. He suffered an amputation to the fingertip with bone loss.

An employee was emptying a trash can into a dumpster. He tripped while turning around, fell over a rail onto the concrete floor, and broke bones in his back and wrist.

During a changeover, an employee was wiping down the dough chunker machine and the chunker closed on his left hand. The employee sustained crushing/laceration injuries, requiring hospitalization and surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
