An employee was assisting the drill rig operator when his left little finger was crushed by the Kelly bar, resulting in amputation of the finger.

An employee was standing about 15 feet away from a concrete truck. As the truck moved up a ramp to dump its load of concrete, one of its tires threw a rock that struck and fractured the employee's left leg. The employee was hospitalized.

An employee was positioning steel casing with a synthetic web sling. As he removed the sling from the casing, the casing shifted and pinched his left middle finger between the casing and the clamp set to receive the next piece, partially amputating his left middle fingertip.

A crew was burning temporary tape during highway construction when a tractor trailer drove into the work crew, injuring three employees. One employee jumped from the roadway into a ditch, resulting in hospitalization with multiple fractures that include the right shoulder, pelvis, leg, ankle, and three vertebrae, as well as road rash, severe facial damage, a brain bleed, multiple cuts and bruises, and shock.

An employee was performing flagging duties for a work zone while standing on the shoulder of a two-lane road. The mirror of a passing pickup truck struck the employee, resulting in injuries to their head, neck, left shoulder, and left arm.

Employees were building a gantry. The injured employee was carrying part of the gantry when their left ring finger was crushed between the gantry and wood. The employee's fingertip was amputated.
