On March 31, 2025, at 10:45, an employee was working on a conveyor when their right arm was amputated at the elbow.
An employee was making adjustments to a vertical bucket conveyor. They were standing at the inspection window, tracking the path of the belt. The belt walked to the side, causing buckets to strike a patch (a steel plate approximately 13 by 6 inches) that was riveted to the top of the conveyor leg housing. The impact ripped the patch from the rivets and the patch fell approximately 80 feet to the ground, striking the employee's head. They sustained a skull fracture.
On October 24, 2018, an employee was cleaning inside a grain bin. The foot peg that the employee was standing on came loose, causing him to lose his balance and slide down the bin slope. He suffered a broken tailbone and was hospitalized.
On or about October 5, 2015, an employee was trying to free the door of a grain dump truck when the locking mechanism got stuck. Once the door was freed, it slammed on the employee's right middle finger. The right middle fingertip was amputated.
