Final narrative
An employee was walking along the catwalk while filling a mushroom growing room and monitoring the operation of the net puller. The employee observed a sideboard of the mushroom growing bed moving at the front of the room and proceeded to investigate. While approaching the area, the employee slipped on compost and instinctively reached out to stabilize himself by grabbing onto the bed frame supporting the net puller. The employee's right hand and arm became entangled in moving ropes in the net puller, resulting in arm and wrist fractures and a hand laceration.
A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.
An employee tripped while walking from one store register station to another. Her forehead struck a register, and she suffered fractures to two vertebrae in her neck. She was hospitalized.
An employee was using an electric pallet jack to retrieve a chemical tote from a staging lane. He inserted the pallet jack under the tote and began to back up. After moving backward for approximately 15 feet, the employee tripped on a collapsible tote. As he fell, the pallet jack ran over his left foot. His ankle was lacerated and his foot was fractured.
An employee had been unloading tires from the back of a pickup truck. The employee went to step down out of the back of the pickup truck and stepped on the lift gate in a vertical position. The employee sustained a subdural hematoma, rib fractures, and a laceration to the spleen.
An employee was on the second level of a mushroom house and nailing a crosspiece of wood to support a mushroom bed when the 2x6 stringer that holds up the 3-foot wide walkway broke and the employee fell to the ground level. The employee sustained two fractured ribs.
