On April 26, 2025, an employee was assisting with a concrete column pour when a 2-inch by 30-foot pipe, which weighed approximately 80 to 100 pounds, moved out of the cage and fell. The pipe struck his right foot and he was hospitalized with a fracture to his right little toe.

On July 18, 2025, a light-equipment operator was using a pressure washer to remove cementitious material buildup from the internal wall of a tremie pipe. The operator was refueling the pressure washer using a 5-gallon gas can. The equipment's engine and exhaust were still hot and the gasoline ignited. Flames spread to the pressure washer skid, a nearby fire extinguisher, and the operator s clothing. The operator sustained second-degree burns to approximately 35% of their body including their hands, lower abdominal area, upper thighs, and knees. The employee was hospitalized.

The injured employee pulled a knife on Employee 2. Employee 2 then went to his truck, got a handgun and shot the injured employee in the lower leg. The injured employee was hospitalized.

An employee was disconnecting two adjoining 4" hoses from a slurry pump when he was splashed by residual hot material left in the hose after a cavitation. He suffered burns to his leg and arm, requiring hospitalization.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was unloading frameless mirrors from a trailer. When unstrapped, the mirrors fell over onto the employee, who suffered a skull fracture, a brain bleed and blood clot, a broken left ankle, and lacerations.

A temporary employee was rigging secondary steel when the steel joist fell to the ground and struck his lower left leg, resulting in fractures to the tibia and fibula.

After unbolting a portion, part of the assembly fell approximately 6 inches onto one employee's left little finger. An employee was stacking a pallet when the pallet fell, injuring her left leg. She was hospitalized and had surgery on her left leg.

Stevenson Crane Service, Inc.

A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

Severo Ortiz Construction LLC

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.

Fence Repair Company

An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.

Beth-Allen Scaffold & Equipment

An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

Sysco Gulf Coast, LLC

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

Norflex, Inc.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
