An employee was unloading a trailer. Cartons of goods fell and struck the employee, who fell down. Cartons of goods (45 pounds each) then fell onto the employee, who suffered a compression fracture to a lower-back vertebra. The employee was hospitalized.

An employee was processing apparel when they stepped through a Z-rack and fell to the floor, resulting in a fractured left hip.

An employee was working in a fitting room. The employee tripped over a long dress and fell to the ground, suffering a broken left ankle.

An employee was decorating breakroom bulletin boards when she fell to the floor, resulting in a fractured arm.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.

An employee was exiting the building and walking down the stairs with co-workers when she fell down the stairs and landed on her left hip, fracturing it.

An employee was utilizing an inclined conveyor belt in the stock room to transport a bathroom rug to the second floor. She was at the bottom of the conveyor belt when the rug fell into the conveyor roller mechanism, jamming it. She was working to clear the jam when her right arm became caught in the 3.5-inch gap between the powered conveyor belt and the underside roller. The employee was hospitalized with six fractures, including her forearm, index finger, ring finger, wrist, and thumb, as well as a severed artery. The machine was not guarded or locked out/tagged out at the time.

Two employees were changing a promotional sign in a store. The plexiglass from the sign fell about 3 feet and struck one of them in the forehead. The employee suffered a brain bleed.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
