An employee was changing out boxed soda concentrate on the soda machine, breaking down boxes, and throwing the broken-down boxes in the trash bin. One of the boxes did not make it into the trash bin. As the employee was turning around to walk away, his foot slipped on the broken-down box and he fell to the floor. The employee was hospitalized with a fractured femur and required surgery.
