An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee in the warehouse was working to pull a reel from under a rack when his finger was crushed between the reel and the rack. The employee sustained a fingertip amputation.

An employee was pushing automotive suspension material using a material rack. The material started to shift and caught the employee's right little finger, crushing it. The employee suffered an amputation to the finger. The finger was re-attached.

An employee was entering a receiving area to start his shift when he was struck by a utility cart being pushed by another employee. The injured employee fell to the floor and suffered a fractured hip.

A temporary employee was moving an I-beam that was on a steel table to the edge of the table. Their right index finger was pinched between the I-beam and the table, resulting in a broken finger.

On November 19, 2024, at approximately 2:40 PM, an employee was using a vertical bandsaw to trim excess material on a life ring. The tip of a finger on the employee's left hand contacted the blade, resulting in an amputation.

An employee was assisting a team member with loading a boat onto a trailer when their right index finger was pinched between the wood bunk and boat dolly, resulting in an amputation between the nail and the first joint.

An employee was operating a forklift when the mast struck a light post. The light post then fell on the employee's head resulting in skull fractures.

An employee fell through a skylight approximately 25 feet to the ground. The employee sustained a possible back injury.

An employee was using a press machine to cut pieces of aluminum when the press came down and crushed his left-hand fingers, resulting in two lacerated fingers and amputation of the little finger.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
