An employee was checking a machine. The machine, though it had been turned off, was still moving; it caught and amputated the employee's left hand.
An employee was operating equipment when a piece of bar got stuck in the rolls. He used another piece of bar to dislodge it. The bar was caught, creating a pinch point between the roll and the bar and resulting in amputation of the employee's left ring finger.
