Final narrative
An employee was unpacking boxes and removing plastic wrap from a pallet that was approximately 6 feet tall. There were also some mirrors in the middle of the pallet. While removing the mirrors, some boxes began to fall off the pallet causing the employee to fall backward to the ground. Boxes did fall on the employee and the fall resulted in a fractured right hip requiring hospitalization and surgery.
An employee was setting up an A-frame shoe fixture. When the employee went to turn the fixture upright, it collapsed on his right index finger, resulting in an amputation.
