A burr on the flange of the link snagged the employee's right glove. The link slid into a seated position and the employee's right middle finger was caught between the top surface of the barge and the top flange of the dogbone link. The finger was partially amputated at the midsection of the distal phalanx.

An employee was working to unload cargo from a ship. A steel coil contacted the employee's left leg. The lower leg was fractured.

While preparing to install a new spool piece for a vapor and spent sulfuric acid line, an employee was working to ensure there was no residual product or pressure in the line before removing the 6-inch hose. As they were performing post-maintenance activities after repairing one of the two discharge pumps, one of the discharge valves would not open. The employee closed the intake pump to cool off the system and open the discharge valve. Two employees opened the inlet valve to return the tank to service.
