Final narrative
An employee was cutting up a large tree that had blown down on a rocky ledge on a hill. He began cutting on the uphill side of the tree, but due to obstructions he had to move to the downhill side. The tree separated from its 7-foot root ball, which started sliding toward him. While attempting to get out of the way, he tripped and fell to the ground, striking his head on an unknown object. As the root ball continued to move toward the employee, he made it below the level of the rock ledge. The root ball rolled over when it reached the ledge and pinned the employee's lower leg, and broke his fibula. He also suffered scrapes and bruises from the fall.

A team was sorting and loading wild horses onto trucks to be shipped to a new location and a gelding that had injured itself was being moved to a single pen. That horse then came back over the top of the employee who was working with it, striking them in the back and launching them approximately 8 feet into the air. This caused him to strike his face on a corral panel. The employee was hospitalized.
On June 5, 2017, at 11:50 AM, an employee was directing a tour of elementary school children and attempting to retrieve biological samples from the Lackawanna River with a net. The employee dropped the net into the river. While attempting to retrieve the net, the employee fell down an 8-foot embankment and landed in the river, breaking three ribs and four vertebrae. The employee was hospitalized.
A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.
An employee tripped while walking from one store register station to another. Her forehead struck a register, and she suffered fractures to two vertebrae in her neck. She was hospitalized.
An employee was using an electric pallet jack to retrieve a chemical tote from a staging lane. He inserted the pallet jack under the tote and began to back up. After moving backward for approximately 15 feet, the employee tripped on a collapsible tote. As he fell, the pallet jack ran over his left foot. His ankle was lacerated and his foot was fractured.
An employee had been unloading tires from the back of a pickup truck. The employee went to step down out of the back of the pickup truck and stepped on the lift gate in a vertical position. The lift gate tipped and employee fell, impacting their side on the upright liftgate. The employee sustained five fractured ribs and a punctured right lung. The employee was hospitalized.

An employee was walking on a trail in a national park and stepped onto an angled and flat slab of granite when her ankle rolled and her weight shifted, but she did not fall. She was hospitalized with a fractured tibia and fibula and a dislocated ankle.

A third employee pulled a locking pin, causing the front wheel mount to move forward into the cab. The cab entrance hand hold crushed the little fingers on the injured employee's hands. The employee was hospitalized and required surgical amputation of the right little finger to the second joint, as well as debriding and stitches to the left little finger.
An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.
On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.
An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
