An employee was descending the side of a barge, having ascended to the top of it to close an opening on top of the barge lid. There was a towing rope on top of the three-step ladder on the barge; the employee put his weight on this rope, believing it to be part of the ladder, and fell backward. His ankle and shin were caught between the top of the third step and the barge lid. He suffered a fracture near the right ankle and another near his right shin and was hospitalized, undergoing surgery.

An employee was preparing a barge for the end of a day's drilling operations. He was setting a spud on the barge when the spud struck him and broke his right leg.

A team of employees was moving boats from one slip in a marina to another using rope. An employee slipped while moving around in a boat and suffered a broken left femur.

AHTNA Marine & Construction

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

MASSMAN CONSTRUCTION CO.

An employee was inserting dogbone links (4 feet 9 inches in length) into the slots between sectional barges to connect the barges. A dogbone link became stuck in the slot and needed to slide down approximately 1 foot to be seated. The employee went to lift the dogbone link up and adjust it when the link became free. A burr on the flange of the link snagged the employee's right glove. The link slid into a seated position and the employee's right middle finger was caught between the top surface of the barge and the top flange of the dogbone link. The finger was partially amputated at the midsection of the distal phalanx.

An employee was working to unload cargo from a ship. A steel coil contacted the employee's left leg. The lower leg was fractured.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee slipped and fell while exiting the cab of his tender truck. The employee was hospitalized with a fractured femur.

An employee fell through a hole in a work platform to the ground about 21 feet below. The employee was hospitalized with multiple fractures and a traumatic brain injury.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
