An employee was working at the disassembly table to remove and replace a hexagonal hub in a steel disk piece from the hot rolling mill. The outer steel ring of the piece collapsed and fell to the ground, striking the employee's foot. The employee was hospitalized for a dislocated right ankle that required surgery.

An employee was installing an adapter plate into a mill stand when the employee's left middle finger was caught between the base of the stand and the 400-pound adapter plate, breaking his finger.

An employee was using a come-along to realign a chain on a sprocket when the employee's left thumb was caught between the chain and sprocket, lacerating and breaking it. The employee was hospitalized and had surgery.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was rolling a pipe in a cutting rack and his right little finger became caught between sections of the pipe. The employee's finger was surgically amputated.

An employee was working a vertical bandsaw, cutting half-inch diameter rods into pieces. While using a steel chisel to move the cut pieces, the blade caught his leather glove and pulled his index finger in. The employee sustained an amputation of the finger between the first and second knuckle.

An employee was operating a lathe in a machine shop. The rotating head of the lathe caught the employee's left glove, resulting in a compound open fracture to the left wrist and a laceration to left ear.

The injured employee was loading a piece of pipe into a pipe bending machine. He had his right hand on the front of the loader while his left hand supported the piece of pipe in the back. A second operator had slightly clamped around the pipe to keep it in place. The third operator called out a measurement. The machine actuated and caught the the injured employee's left middle and ring fingers between the wiper and the material. The employee required surgery and sustained amputations to the distal joint of the middle and ring fingers.

On May 17, 2025, at 8:30 a.m., an employee was placing a wooden chock under a pipe to secure it in place for welding. The combined weight of the pipe and chock caused the chock to crush the employee's left ring and middle fingertips against the floor, resulting in hospitalization.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
