An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was mixing self-leveling cement when his hand was caught in a mixer. He suffered a partial amputation to his left middle finger, as well as a laceration to his buttocks.

An employee was working to pull open the stuck door of a concrete slurry container. The door sprang open and struck the employee in the lower jaw, causing a laceration and fracture to the jaw.

An employee was stepping backward through a framed wall when they tripped over the base metal track and fell to the ground, resulting in a broken arm.

An electric crew was unloading a lighting instrument rack (approximately 400-500 pounds) from a box truck when one of the wheels rolled off the lift gate and the rack tipped over, falling onto the injured employee's right leg. The employee was hospitalized with a fractured tibia and required surgery.

An employee was unloading a dump bin when something became stuck in the wheel. The employee went to dislodge the object when the bin rolled back and caught his right middle finger between the bin and the wheel. The employee sustained a partial amputation of the finger.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
