AbbVie Inc.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A maintenance employee was moving a forklift from the loading dock when the forklift fell off the dock. The employee was struck by the overhead portion of the roll bar, injuring his right knee. He was hospitalized and underwent surgery.

A yard truck pulled the trailer away from the dock and the PIV fell backward out of the trailer. The employee struck their back on the headache rack of the PIV and was caught between it and the dock plate. The employee was hospitalized with a blunt force injury.

An employee was transferring scrap metal from a tote into a metal recycling dumpster for disposal. While throwing a 20-pound stainless steel motor into the dumpster, the employee moved in closer to ensure it went inside. As they tossed the motor, it appeared to be off-target, so they went to push and redirect it while it was in the air. During this action, their right ring and little fingers were caught between the falling motor and the edge of the dumpster. The employee suffered a crush injury to the ring finger and an amputation of the little finger above the top knuckle.

An employee was emptying a waste barrel into a steel hopper. The barrels are attached to a forklift with a bracket and the forklift is used to raise the barrel over the steel hopper. Once raised, a chain is used to operate a pulley mechanism attached to the bracket that tips the barrel horizontally to empty the contents into the hopper. A link in the chain used to operate the pulley system detached. The employee went to manually dump the barrel and the barrel slid down, crushing his left hand between the barrel and the hopper. The employee sustained an injury to their left ring and little fingers that required surgery and partial amputation of the little finger.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
