An employee was performing annual preventative maintenance within a switchgear unit. The employee contacted an energized part on the unit, resulting in burns to his hand and arm.

An employee was descending a 10-foot A-frame ladder. The ladder came out from under the employee when he was about 5 to 6 feet above ground level. He fell, landed on the left side of his body, and sustained fractures to four ribs, his pelvis, and hip socket, and sustained injuries on the left side of his body. The employee was hospitalized.

An employee was handling a small platform ladder with three steps and two wheels in the front. The employee was found on the floor underneath the ladder. She was hospitalized with multiple fractures to the sacrum.

An employee was on a ladder four feet above the ground to install insulation. The ladder began to slide and the employee jumped to the shop floor, breaking his left tibia and fibula at the ankle. He was hospitalized, requiring surgery.

On July 21, 2025, an employee was working from a 10-foot step ladder while installing a 3-by-5-foot glass window pane on the outside of a new home under construction. The window fell from the frame and the employee went to catch it when the ladder slipped. The employee fell 5 feet to the ground and landed on small pieces of concrete left over from the foundation pour. The employee was hospitalized with a dislocated elbow and fractured wrist that required surgery.

An employee was working from a baker scaffold while installing ceilings at a construction site. One of the scaffold wheels rolled into a hole in the floor and the scaffold tipped, causing the employee to fall approximately 4 feet to the concrete below. The employee's head struck the concrete, resulting in a head injury.

An employee was removing parts for replacement during equipment maintenance. His right forearm was exposed to a mixture of silicon dioxide (silica) and trichlorosilane that caused an exothermic reaction and chemical burns. The employee was hospitalized.

An employee positioned a pallet jack under a mobile TV stand to lift the stand and then clean the wheels. As the employee raised the pallet jack, the TV stand fell forward toward the handle of the pallet jack. The employee went to catch the TV stand and their right thumb became caught between the TV mount frame and the pallet jack, resulting in amputation.

An employee was working on the crimp machine when their left middle finger became caught in the machine. The employee sustained the amputation of the fingertip through part of the nail and was hospitalized. The machine was guarded at the time.

An employee was greasing the belt of an air handler and manually moving the belt when their left ring finger was caught between the belt and the pulley. The employee sustained a severe laceration to the fingertip that resulted in an amputation.

An employee was moving a heavy-top table for a probe station using a dolly. The dolly got caught on a piece of carpet while crossing the threshold of a door and the equipment slid off the dolly. The employee's right hand was pinched between it and the floor, resulting in a fingertip amputation and severe lacerations to two other fingers. The injuries required surgery.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
