An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee had been loading packages into a vehicle. While exiting the vehicle, he tripped and fell, fracturing both ankles.

An employee was exiting a train. The employee's foot was caught, and the employee fell, suffering a broken left arm.

An employee was dropping customers off with a shuttle. He stepped out of the shuttle and his foot entered a pothole, causing him to twist his ankle and then fall to the ground. The employee sustained multiple dislocations to the ankle requiring surgery.

An employee was approximately 5 feet up on a ladder while drilling a hole for cable installation. A wasp attacked him. As he was coming down the ladder, he landed flat on his feet on the ground, and his right leg was twisted. The employee sustained a fractured tibia.

The employee shut off a steam valve and was descending a ladder to enter a berth pit and open the cargo line valve. He slipped during the descent causing his right foot to be submerged in the hot water inside the dock pit. Hot water entered his right boot resulting in burns to his right foot/ankle.

An employee was on a set of movable metal steps when boxes on a dolly below him fell and struck the steps, knocking them out from under him. The employee fell on the steps and was hospitalized with a collapsed lung and multiple rib fractures on his right side.

An employee was walking inside when she fell on the floor and broke her femur.

On 9/28/2020, an employee fell from a ladder while reaching to perform ceiling work and injured his hip. The injury did not require medical treatment. On 10/04/2020, the same employee reinjured his hip while moving a couch and was hospitalized for treatment.

An employee was returning to her work area within the office using crutches. Her crutch slipped on a tiled portion of the floor. The employee caught herself from falling with the leg that had the pre-existing injury. She was hospitalized for muscle trauma to the upper right leg.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
