An employee was removing a hose from a chemical tank after filling the tank with hexamethylenediamine (HMD). He checked the line to ensure that the hose was clear, then disconnected the hose at the connection. Residual chemical sprayed onto his face, causing a second-degree burn. He was hospitalized.

A shipping/receiving operator was working on a loading rack, loading a railcar. A 2-inch discharge cap failed and released pressure along with sulfuric acid from the railcar. The acid sprayed onto the employee, who suffered burns to the chest and the lower facial area. The employee was hospitalized.

An employee was pressure washing truck scales when they were splashed with water and an unknown chemical on their leg and immediately experienced a burning sensation. The employee was hospitalized with a chemical burn and required surgery.

An employee was on a ladder disconnecting a 1-inch hose, known to have last carried acrylic acid that had been drained. When the hose was disconnected, residual acid dripped onto the employee's shoulder/arm area, causing a second-degree chemical burn.

Employees were removing scaffolding that was used to repair an ion exchange tank. The line was undergoing the regen process when a piece of scaffold struck a PVC pipe that transported hydrochloric acid (HCl). The injured employee was placing material inside a scaffold rack when he was sprayed in the face with HCl and sustained chemical burns to his eyes, face, chest, right arm, and left leg.

An employee was servicing a bypass feeder system. The system released hot water onto the employee, resulting in first- and second-degree burns to his face, chest, right arm, and both hands.

An employee was cleaning a formic acid hose with water when formic acid splashed his right shoulder, both arms, and his face under his face shield. He was hospitalized.

A driver had just climbed out of a truck. He stepped back to close the door and his foot went into a low spot on the ground. He fell backward onto an incline, rolled, and suffered a compound fracture to the wrist. He was hospitalized, requiring surgery.

An employee was walking between two tanks when he stepped into a sump pit that was missing its metal grate. His leg was submerged in sulfuric acid, and he suffered chemical burns to the lower part of his right foot. The employee was hospitalized.

On June 24, 2025, at 2:11 p.m., an employee was about to unbolt a spool connected to a vessel flange that needed to be removed for another crew to clean it. The employee saw leaking chemical product. The employee began running away from the product when he slipped and fell into the product, resulting in chemical burns to his arms and legs. The employee was hospitalized. Personal protective equipment (PPE) was not worn at the time.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
