Final narrative
On August 7, 2025, employees were using forklift forks to move a single-wheel slitter so they could inspect a broken leg on it. While they were sliding the slitter off the forks, it fell, trapping an employee s left ring finger against the concrete. The employee suffered a severe avulsion to the finger, which was then medically amputated at the knuckle.

An employee was attempting to roll a 1,300-pound coil that was 3 inches wide and 4 feet high when the coil fell on the employee's left ankle, causing broken bones and lacerations.
An employee was working on a flatbed semi-trailer that could extend its length. The air cylinder to release the pinning device was not working properly, and he manually released it. When the truck pulled forward, the employee was caught between two pieces of steel. He suffered broken ribs and punctured lungs.
An employee was moving scraps on an assembly line. She was sitting down beside the conveyor belt with her hand resting on the conveyor belt. The conveyor belt is automatic and turned on. Her cotton glove was caught in the belt, resulting in the left index finger getting caught between the chain and the frame. She was hospitalized with a lacerated/broken left index finger.

The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.
An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.
An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.
On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.
