An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was riding a four-wheeler while moving cattle into another pen when they were thrown off the vehicle and landed on the ground. The employee sustained fractures to their upper arm, pelvis, and right leg.

An employee was moving a stand-up powered industrial truck from a warehouse to a service area. During a turn on a slight decline, the vehicle's steering chain broke and the vehicle spun. The employee fell off the vehicle and landed on his right knee, breaking his fibula.

Two employees were operating a paving machine at approximately 1 mile per hour. The paver made a sudden turn, that threw the injured employee off balance. He reached for the handhold next to him and missed, falling 6 inches off the platform to the ground. The adjustable handle for the wing on the paving machine went between his left-hand fingers with the full weight of his body. The employee suffered a closed fracture of the fourth metacarpal bone in his hand.

An employee slipped on some spilled sauce in the walk-in cooler and fell, hitting the front of his head on the concrete. Approximately 30 minutes later, he became ill, collapsed, and had a seizure. The employee was hospitalized with an intraparenchymal hemorrhage (IPH).

At about 5:50 a.m. on August 14, 2025, an employee was changing tee cups on a golf course. He stepped away to the brush to grab a branch from the ground and a rattlesnake bit the top of his right hand.

An employee was trimming hedges when he slipped and his right thumb contacted the blades of the hedge trimmers. He sustained amputation of approximately 80% of the right thumb beyond the first joint and a laceration to the right index finger.

On July 28, 2025, an employee was removing ice buildup from the floor of a walk-in freezer. He was melting the ice with warm water when he slipped and fell on the floor. The employee was hospitalized with fractures to his left leg/ankle and he required surgery.

An employee was exiting the building when her golf spikes caught the floor mat causing her to fall. She was hospitalized with lacerations to her head, face, hands, and arms.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
