An employee was raising an electric ladder to retrieve merchandise from overhead racking. As it rose, the railing of the machine caught his right middle finger against the bottom of the high bay netting supports, causing an amputation just below the fingernail.

The excavator was outside of the trench and more than 25 feet away when it moved a pipe, pinching the employee's right arm between the two pipes. The employee's forearm was fractured.

The employee also sustained multiple fractured ribs and fractured vertebrae.
