On March 16, 2023, an employee was removing plastic and straps from palletized material. As she pulled the strap out from under the pallet, it broke, causing the employee to fall backward and sustain a fractured pelvis. The employee was hospitalized.

An employee was crossing a materials aisle, moving away from her welding cell, when a lift truck struck her as it backed up. She suffered an injury to the right foot.

An employee was performing maintenance activities on a drive train when they were caught between a tensioner and a beam, resulting in fractured ribs and a fractured sternum.

An employee amputated the their left index fingertip while loading a weld fixture into a weld robot cell.

An employee was mowing a residential lawn at a client's house. The mower slid on the land and tipped over onto him. The employee was hospitalized with a fractured vertebra in the neck and partial paralysis.

An employee was driving a forklift when its right front wheel dropped off a 6-inch curb. The forklift rolled over and trapped him against the concrete surface. He suffered a broken pelvis, ribs, and collarbone, as well as a collapsed lung.

An employee was operating a utility task vehicle (UTV). During a left turn, the vehicle rolled over on its right side. The employee suffered a lumbar strain and fractures to four lower-back vertebrae. She was hospitalized.

An employee was loading a telehandler onto a trailer and the right wheels were misaligned with the trailer, causing the telehandler to tip and fall to the right. The employee went to exit the telehandler as it fell, striking his head on the cage and frame of the telehandler. The employee sustained a concussion, a head laceration, and fractured vertebrae.

An employee was driving an ATV when it overturned and rolled onto its side. The employee sustained a fractured elbow requiring hospitalization and surgery.

An employee was cleaning mud out of the tracks of a mini excavator while the operator had the machine tilted up off the ground to one side. The tracks moved and the employee's left wrist was caught in the gears. The employee sustained puncture wounds to the wrist and two fractures.

An employee was loading a truck with a forklift. The truck drove away from the dock and the forklift fell between the dock and the truck. The employee was hospitalized with injuries to their head, back, and leg.

An employee was moving trucks in the yard. He was working between trucks when a back door swung open and caught their right hand against the frame of another truck. The employee sustained a crushing injury to a finger.

On June 9, 2025, an employee was walking in the yard and parking area. The employee stepped up from the yard/parking lot onto a patio with an approximate 6" rise at the exterior corner of the main office. After his second foot was planted on the patio surface, he lost his balance and fell backward to the concrete ground. The employee was hospitalized with a broken hip that required surgery and a laceration to the elbow.

An employee was working in a ditch while a mini excavator was digging in front of them. Teeth on the excavator bucket struck the employee's left foot, resulting in a fracture.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
