An employee was struck by a pallet that fell from an empty pallet storage bay. The employee sustained a dislocated shoulder.
An employee was cleaning around a cog belt when their rag got caught in the belt and pulled the employee's hand into the belt resulting in amputations to two of the employee's fingertips. The cog belt was not locked out/tagged out at the time.
An employee was using a release handle extension to disconnect the fifth wheel of a converter dolly from a trailer. The employee slipped and fell backward, landing on her left hand/wrist and resulting in a fracture.
An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.
