On August 27, 2025, a service technician was descending a ladder at a service call when his right foot slipped off the ladder rung. The ladder rotated as his weight shifted. The employee lost his footing and fell approximately 8 feet to a concrete lower level. He sustained fractures to his left tibia and left ankle.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below.
