An employee was removing product from a leg quarter saw with a hook when the hook became jammed. As he was getting the hook free, his right hand came into contact with the saw, which partially amputated his middle finger.

An employee was working with a golf ball washer machine. He was cleaning a belt on the side of the machine using a sponge. The sponge got caught by the rotating belt and was pulled in. The employee's right hand was pulled in as well and he sustained an amputation to a finger below the nail.

A temporary employee was working to remove a detached piece of rubber EPDM product that was hanging on the outside of a feed chute. As the employee grabbed the elongated material, the material reengaged inside the machine above the feed chute and pulled back, pinching their left index fingertip. The injured employee was working alongside a trainer. The trainer was adjusting the side fences that hold the wood in place as it passes through the molder. The injured employee was using compressed air to clean the area. They went to remove a sliver of wood to prevent a potential jam. The rollers activated, which processed material through the machine, caught the employee s glove, and pulled their hand toward the cutting head of the molder. The employee sustained lacerations to their right middle and ring fingers and an amputation to the distal phalanx of the middle finger.
