Final narrative
An employee was walking to collect trash from a box at a work cell parts stand. A leg of the stand caught the employee's foot, causing the employee to lose balance, fall to the floor, and suffer a broken left elbow.
An employee was walking in a metal welding area. A forklift struck the employee in the left leg, causing a laceration and fracture to their left lower leg/foot. The employee was hospitalized.
An employee had just finished connecting a chain and hook to a steel mold D-ring in order to remove the mold from a plastic injection molding machine. As the employee was adding tension to the chain, the D-ring snapped, causing the hook to strike the employee. They sustained lacerations to the head and nose, as well as fractures to the nose, forehead and right cheek bone.
An employee was working on an assembly line. They stepped into a 4- to 6-inch deep gap/hole, lost balance and fell, striking their side against the machine.
