An employee was using a screw to connect an additional metal support bar to the support bar installed on the mill for maintenance purposes. Once the additional bar was connected, the mechanics removed the mill face and screen from the housing. The screw failed and the mill face and screen fell. The employee's hand was caught between between the mill face/screen and a support bar and a finger was partially amputated.

ARCH WOOD PROTECTION, INC.

An employee was removing a vessel cap when 60% chromic acid solution sprayed out of an open valve and contacted him, causing third degree burns on his shoulder, back, and wrists. He was hospitalized and had surgery.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

Rae Corporation

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

Red Dot Corporation

An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.

He checked the line to ensure that the hose was clear, then disconnected the hose at the connection. Residual chemical sprayed onto his face, causing a second-degree burn. He was hospitalized.

Two employees were moving a railcar (plastic pellet hopper car) using a mobile railcar mover. One employee was the engineer in control of the railcar mover and the injured employee was a switchman. The railcar mover went forward and the hopper railcar struck the injured employee, resulting in a severe crushing injury to the employee's right arm.

An employee was opening a filter housing in the potassium chloride production area and got sprayed with a hot solution of potassium chloride brine. The employee sustained thermal burns to their right hand and legs.

An employee was performing maintenance on a guillotine cutter machine. As the employee was placing a pin in the machine, his left middle fingertip was caught, resulting in a partial amputation to the finger. The machine was not locked out at the time.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.
