Two employees were hooking up a hydro excavation trailer to clean out water troughs. The injured employee was guiding employee 2 while she was backing up the truck to the trailer. The injured employee then began lowering the jack of the trailer to connect the trailer to the ball hitch on the truck. The trailer would not come down onto the ball hitch. The injured employee was manually adjusting the trailer when it came down and caught fingers on his left hand between the trailer s tongue and the ball hitch resulting in fractures.

Employees had shut down and locked out a cotton gin lint cleaner to clean it. An employee entered the lint cleaner and his glove was caught by the teeth of the cleaner. His glove was pulled off and the employee sustained a de-gloving injury to the top of the right hand.

An employee stepped out onto the damp porch, slipped, and fell injuring her left knee.

An employee was walking through the hospital pen and a cow struck him with her head. The employee sustained an orbital fracture.

An employee was unloading cattle from a pen to the main drive alley. While the employee was inside the pen, a cow rammed her and knocked her down. The employee sustained a contusion to her right upper torso and three fractured ribs.

The employee's right ring finger was severed and required surgery to reattach the severed portion of the finger. The employee was hospitalized.

An employee was helping to remove attachment forks from a loader. The attachment failed to come off. The employee went to shake the attachment as the loader lowered the forks. The employee's left index finger became caught under the attachment, resulting in a fingertip amputation.

A chain attached to the head restraint device caught the injured employee's index finger, resulting in a fingertip amputation around the fingernail area that required surgical amputation at the first knuckle.

An employee was sorting cows in a pen when a cow struck a gate the employee was standing behind. The gate subsequently struck the employee, causing him to fall backward and hit his head. The employee suffered a closed head injury that required hospitalization.

An employee was servicing a master lift station and setting up a power analyzer when an arc flash occurred. The employee sustained burns to their hand, arm, neck, and face.

An employee was moving cattle out of a pen via horseback. The employee's horse lost footing and fell on its side with the employee still on it. The employee was hospitalized with a dislocated right collarbone and eight fractured ribs.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
