An employee was pushing a jammed pizza case down a conveyor system. The employee's left hand was caught between the case and the end stop of the conveyor; the employee suffered fractures and lacerations to the little, ring, and middle fingers.

While operating a cheese shredder in auto production mode, an employee's right index finger was crushed by the pusher arm.

On September 21, 2017, an employee was placing labels on pallets when a forklift ran over his foot.

An employee was performing preventive maintenance on the line 2 case erector, a machine that forms corrugated into a box. The employee was looking to see if a bearing was running smoothly in the machine and had a shop rag in his right hand. The rag got caught in the moving chain and sprocket assembly, pulling his right index finger into the assembly. The finger was severely lacerated and contorted such that it was amputated at the hospital.

An employee's finger was caught in the premelter conveyor while clearing pizzas off the conveyor. The fingertip was amputated.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

The employee was cleaning the black substance from the discharge hole of the rotary when their fingers were struck by the rotary cups. The employee sustained amputation injuries to the middle and ring fingers on their left hand.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.
