On May 23, 2024, an employee was strapping freight inside of a trailer when he fell out of the trailer. The employee sustained head trauma and injuries to his left big toe and right elbow.

An employee was preparing to offload a trailer that was backing up to a dock. As the employee opened the trailer doors, a 300- to 400-pound steel rack containing parts fell onto the employee, pinning him to the ground. The employee was hospitalized with a pulmonary contusion to the right upper lobe and fractures to the T12, L1, and L2 vertebrae of the thoracic and lumbar spine.

An employee was getting out of a truck cab. Her right ankle gave way and she fell, landing on her back. She suffered a broken right ankle, as well as a possible back fracture.

An employee was unloading freight from a truck. The pallet holding the freight slid and the lift gate came down on the employee's left foot, breaking the foot and the ankle.

An employee was cleaning a truck in the rain. As the employee began to step down on the passenger side of the truck, she fell. Her back struck the ground. She suffered a closed head injury, a lumbar strain, a thoracic strain, and pain in both ankles.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was picking bulk bags for shipments. While cleaning pellets off the ground, a stacked bulk bag (full of plastic pellets) collapsed and fell on the employee. The employee sustained fractures to their ribs, pelvis, and left femur.

An employee was delivering goods to a customer. He was folding the truck's lift gate when his left middle finger was pinched in the lift gate, resulting in an amputation.

An employee was operating an electric pallet jack, delivering pallets of groceries to a store. The employee was walking backward when her foot was pinned between the pallet jack and a video pole, resulting in lacerations and fractures to her ankle and heel. She was hospitalized.

An employee was tightening a turnbuckle while attempting to conduct lash-back operations on a ship when their left gloved hand was caught between the upper and lower turnbuckles. The employee suffered a crush injury to the little finger that resulted in a fingertip amputation.

An employee was unloading a concrete utility pole from a log loader to place in a pole rack for storage at the delivery site. The employee raised the pole. The pole spun around and released from the grapple/boom and fell, striking the employee. The employee sustained fractures to their arm and pelvis.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
