An employee was sitting at his desk when he felt itching and pain in his left-leg shin area. He had received a poisonous spider bite and was hospitalized with a 105-degree temperature.

An employee slipped and fell, resulting in head injuries.

The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

An employee opened a flammables storage cabinet to get something out, smelled a chemical odor, and began to feel unwell and dizzy. She was hospitalized, having been exposed to formic acid from a leaking bottle at the bottom of the cabinet.

The injured employee was cleaning a pre-mix blend from a high-speed mixer. Another employee working in the same room opened the pneumatic valve for the machine. The injured employee's right forearm was crushed in the mixer when the discharge valve closed. He was hospitalized.
