An employee was on a step ladder retrieving a new automotive radiator from a stock room shelf. While descending the ladder, he missed the second step from the bottom and fell back. The employee struck his head and neck on the shelving behind him and was hospitalized.
An employee was delivering supplies to the store and putting them away when the employee tripped over a tote of materials, falling to the floor and fracturing the right shoulder.
