The employee was assisting a team of paint technicians with cleaning out the black e-coat tank. The employee was standing on a platform next to the tank that had safety guard rails in place while adjusting a large hose that was down in the tank.

What happened

The employee was standing on a platform next to the tank that had safety guard rails in place while adjusting a large hose that was down in the tank. When pulling shaking the hose in an attempt to adjust it causing the safety gate to be dislodged out of the channel where it snaps into place. The safety gate fell forward into the tank and the employee fell into the tank following the safety gate.
