Associate was cleaning the holding tank - 902 when the tank lid fell on the associate left arm.

What happened

the holding tank-902 in the Prewhip room was clogged and [REDACTED] climbed up to the tank to checked the inside of the tank. There was a hose in the tank already and cleaning was on-going when [REDACTED] opened the tank and the hose inside pushed forward to the direction of her face. [REDACTED] reached to hold the hose with her left hand and the holding tank lid fell on her left hand. [REDACTED] was not holding the lid down with her right hand when the lid fell on her left hand.
