Before the incident

The employee was getting ready to remove a worn tool from the [REDACTED] machining center so that they could replace it with a new tool.

When the employee went to press the tool release button they pressed the tool rotation button instead which caused the tool to rotate clockwise. This caused their glove to get wrapped up around the tool pulling their hand into and around the tool.
