Before the incident

EE was holding a black foam pad in between the fork of the [REDACTED] and the [REDACTED] equipment as they were attempting to lift the equipment to reposition to Line 4 production. [REDACTED] operator called clear all Maintenance Techs cleared hands except for injured EE who did not clear hand in time resulting in index and middle fingers being caught between [REDACTED] and black foam pad with upward motion from [REDACTED] fork. Injuries resulted in fractures of index and middle finger with nail nailbed avulsion.

What happened

The forklift was lower to release EE’s hand from being trapped between ABC and forks. First aid was administered to EE. WorkCare Telehealth was engaged to provide medical evaluation. EE sought medical care at Temporary Agency’s Occupational Health Clinic.
