Before the incident

The extrusion line 341 went down due to a possible winder failure (the roll wobbled and fell off the core chuck) the team was in the process of starting the line up EE was on the south side of the line between the scanner and the s wrap EE was helping another employee pull the top sheet out of the s wrap while the other employee was cutting the sheet While EE was pulling the sheet out of the s wrap his left forearm came in contact with the other employees level 3 blade (in a swiping motion cutting across) EE sustained a laceration to the left forearm as a result
