Before the incident

There were 2 pallets on the floor next to each other. One pallet had 55 lb bags of finished product in it (i.e. rocks sand). The Employee pulled one bag close to her then lifted the bag up taking a step backwards she lost her balance as tripped over the empty pallet sitting on the floor.

What happened

As the employee stubbed on the pallet while walking backwards and carrying a 55 lb bag material she fell down and tried to brace herself with her right hand.
