EE experienced a ground level fall on [REDACTED] in the assigned workstation near [REDACTED] EE provided a written statement as well as witness statements (attached) as the EE tripped on an airline in the work station The incident was not reported until [REDACTED] as the EE was prescribed PT as follow up to continual pain EE stated that it was reported late because they thought it would improve and there were good days and bad days However the pain now is constant and has worsen On [REDACTED] EHS manager escalated this incident to a recordable injury
