Before the incident

Injured was driving a jack in reverse down the dock

What happened

Associate was driving the jack down the shipping dock. After refueling the equipment associate stopped and started driving the jack in reverse back towards the returns area to drop off a hand scanner. As the associate attempted to turn the jack to face the returns area it looked as if associate was thrown from the jack. associate stated it happened so fast one moment on the jack the next on the floor.
