Before the incident

Unwinding a reel of plastic sheet.

What happened

The screws holding the sheet to the reel had popped off due to the tension energy from the sheet. After the plastic let go it unwound under the reel and toward the employee. The employee put his hands up to protect his face. The screws had remained in the sheet and when it whipped down toward the employee the screw of the sheet struck his left hand puncturing the lower thumb palm area.
